If you want photocopy of any of your people's SS-5 forms (Application for Social Security Number / Card), just go to RootsWeb's "advanced search" Search the Social Security Death Index (SSDI) and type in their Social Security Number (or use the advanced search feature to find their Social Security Number), then click on the SS-5 Letter link and a letter will be generated for you to send off for a copy. Use one form per person. You may submit more than one request at a time, but don't overwhelmed them.

When you receive the photocopy (in 8 - 10 weeks or longer), it will look like something like this: Henry Lester McCLENDON, who may be related (for example, a brother) to Leon Frank McCLENDON's grandfather, Leon Frank "L. F. or Lonnie" MCCLENDON. Sometimes, however, you'll receive other documents related to request for benefits, etc. instead of the SS-5 form. If you know who this Henry Lester McCLENDON is, please contact me. Thanks. Pat

A word of caution: If you mess up in this process you are band from the service. Each time you submit your web site, you will get a number via e-mail that you that you need to type into to dialog box before it hitting the submit button. If there is a problem (your server timed out, etc.), you will told to try again. DON'T USE THE SAME NUMBER, instead wait to get a new number via e-mail. I used the old number and I can't get my http://www.PatMcCendon.com/ indexed by them. I'm trying to get in touch with them, now.
