Harold Graham sent me this file in Microsoft Word format. (I converted into a web page using Microsoft Word 2000.) He said that he was the “compiler” since, in addition to his own research, he has used the research of others. There is a bibliography that he usually tags on to the end, but he did not send it since it is on another file. He offered to send it and I’ll take him up on it and well post it after I get it.
